Coordinator, Home Based Palliative Care-Karachi

Aga Khan University Hospital, Karachi                                  Qualification:

• Bachelor’s degree, Master’s degree would be preferred. 

Experience:

• Minimum 3 to 4 years of relevant experience, preferably in health care 

  Duties & Responsibilities:

• Ability to assess, plan and coordinate.
• Facilitates communication among members of multidisciplinary palliative care team to prevent fragmented or delay care that could adversely affect patient outcomes especially in discharge planning.
• Coordinate with support services such as PBSD, safety and security, pharmacy etc and Identify potential patients, plan their visits and explain home health services
